The Foundation offers up to three potential $1000.00 grants to CSNO members and their schools for a study or project related to one or more of the following focus areas of the Foundation: 1) enhance school nurse visibility; 2) promote school nurse achievements; 3) emphasize collaboration; and 4) support school nurse data collection.

5. Feasibility: Describe activities for your project or research to support the attainability of your goal(s), including the potential number of children who may benefit from your project.
6. Budget: Please describe the proposed project or research cost-effective use of funds (each grant is set at $1000.00, with a rare potential for slightly higher funding as available), along with rationale and justification for each budget item:
a. Include specific planned costs associated with activities of your project, along with how each cost relates to the current focus of SNOCF. (Examples of acceptable use of funds could include costs for printing, software, supplies, participant incentives, other research or project support, etc.)
b. Include any specific vendor price quotes or other pertinent information regarding specific project costs.
c. Include any personnel costs (non-salaried team facilitation, secretarial, data analysis, statistical computation, etc).
7. Project Timeline: Please include specific planned dates for the beginning, intermediate, and completion phases of your project. Explain how the project timeline is linked to the budget timeline.
8. For the SNOCF grant committee to follow your project goal projections while in process, describe your plan to monitor and measure Spring 2026 progress, and the final status of your project at completion.
9. Indicate the type(s) of tools you will utilize to measure outcomes and evaluate your project (such as data collection, surveys, questionnaires, interviews, post-tests, return demonstrations, power points, presentations, letters from faculty, administrators, community, etc.).
10. List the specific support and collaboration you may receive from your school and/or district’s administrators/other individuals/groups/organizations for this project.
